Membership Pricing
$150 per month
Or $1,800 per year
$150 One Time Enrollment Fee
For adults 19 years and older
Monthly, quarterly, semiannual, and annual billing options are available.
Care Included With Membership
Visits and Physician Access
- Comprehensive initial neurological examination lasting 30 to 60 minutes
- Neurological examinations twice yearly
- Unlimited office visits
- Same day or next day acute visits
- Direct access to the neurologist
Communication and Continuity
- Online access and messaging
- Telemedicine
- Medical record review
Referrals and Patient Education
- Referrals for diagnostic laboratory testing and radiology
- Referrals and advocacy involving primary care and other specialists
- Clear explanations of medical and neurological conditions
Insurance and Medicare
Neuro Life is a Direct Specialty Care practice. Membership is not health insurance and does not cover hospitalization, surgery, or other outside medical expenses.
Dr. Gamez does not bill Medicare for his professional services. Medicare beneficiaries enter the applicable private contract before receiving care.
Coverage for prescriptions, outside testing, and other services depends on your insurance plan and applicable providers.
